Choose the form that applies to you. Adults 18 and older should complete the Adult waiver. For participants under 18, a parent or guardian must complete the Student waiver.

$50 Refundable Deposit Required: A $50 refundable deposit is required before completing your waiver form. This will be refunded at the end of camp.

Adult Waiver

18 and Older

For participants who are 18 years of age or older. Complete and sign on your own behalf.

Student Waiver

Minor Under 18

For participants under 18 years of age. Must be completed and signed by a parent or legal guardian.
